Living well with Long Term Conditions

We want to support residents with long term conditions to live independently, so they can live well for longer. If you've had a recent diagnosis of a long-term condition, or a deterioration in an existing condition, there's lots of things you can do to and support you can access.

Equipment, Adaptations and Technology-Enabled Care
Equipment, adaptations to your home and Technology-Enabled Care can help you to remain independent in your own home. There’s lots of different options, available from both Manchester City Council and external providers. Visit the Equipment, Adaptations and Technology-Enabled Care page to find out what's available and how it may benefit you.

Help and Support Manchester – Directory of Providers
Help and Support Manchester's Directory of Providers contains the details of hundreds of community support groups and charities. You can browse the Long-Term Health Conditions services.

Re-ablement
You may benefit from 'Re-ablement' – which is a short-term, intensive package of care to help you regain your confidence and independence at home. Our specially trained staff work with you to identify your goals, then use support, guidance, learning and often small pieces of equipment to help you manage.

Not everyone is suitable for Re-ablement – please get in touch with our contact centre to find out more on: 0161 234 5001

Condition-Specific Support
The NHS website is a great resource to visit for specific advice of different long-term health conditions.

Your GP may also be able to give you advice and sign-post you to different resources and support.
