There is no requirement to sign up to the register - it is up to you.

What happens to the information
The information you give us will be held securely by the Local Authority until the child turns 19, then it will be archived. Please also refer to the Privacy Notice on the form.

If you decide at a later date that you would like your details removed from the Register, you can do so by emailing Your details will be removed within 30 days.

How do I put my child's details on the database?
Registration is to be completed by, or on behalf of the child or young person who has a disability (for example, a parent or carer could complete the form). A separate form will need to be completed for each child in the family who is disabled.

To register, please complete the register of disabled children and young people form.
