Shared Lives services
There are several types of service provided:
This model is designed to offer support to customers who may benefit from living with single providers or with the provider's family. The minimum requirement is that each customer has their own bedroom. Customers live with a provider on a day to day basis in an ordinary domestic setting. This promotes integration into community living and participation, and can help develop skills to become more independent in the future.
As a care provider, you may be entitled to respite services based on the assessed needs of the customer you are supporting. Some customers are also assessed as requiring day time support outside of the Shared lives placement.
This model provides services to vulnerable adults who do not need someone with them most of the time. Support can be provided by people living at the same address or off-site but close by.
Supported Lodgings providers offer accommodation, breakfast, lunch and an evening meal as well as some ongoing support and maintaining a link with the Shared Lives team.
Short Term Care (Link or Family Respite)
In this model, the customer may stay in your home for periods of a night to a few weeks. In this model we try to place the same customer(s) with the same providers each time for consistency and continuity of service.
This model delivers social care typically in two to three hour sessions. It's designed to develop the customer's skills, confidence and independence. The sessional worker may support the person with their personal care needs, help them to maintain an active and healthy lifestyle, or enable them to socialise, either at home or out in the community.
Please note Shared Lives is a citywide service
